Regarding, your General and Dental Health, we collect: histories of medical, dental, and health; Treatment Plans , Notes, Details, Correspondence , Fees, Payments, and Clinical records pertaining to your care, from all of your past/present treatment providers (inclusive of X-rays, clinical photographs, and digital scans pertaining to your treatments)

For What Purposes?
